RE-ENTRY PROGRAM

OUR MISSION

Through our programs we ensure that fellow peers receive advocacy, education, and mutual support. We
strive to build human spirit and self-esteem by acceptance, working together and opening doors.

COMMUNITY PEER SUPPORT SERVICES

RE-ENTRY PROGRAM RESOURCES

- Benefits – assist individuals in obtaining benefits, such as Social Security, TDAP, TCA, Food Stamps, Medical Assistance, Housing, Shelter.

- Employment assistance - working with individuals one-on-one with completing applications, creating resumes and practicing mock interviewing skills. Also, assisting to connect individuals to employment services.


- Education assistance - assisting individuals with obtaining their GED or applications to attend college.

- Health Services – assist individuals in obtaining mental health, substance abuse and primary care health services


- Direct Peer to Peer Support - individuals will work one on one with a CPSS Advocate to establish goals, identify challenges, strengths and develop a plan of action

PEER DIRECTED SUPPORT

Support and guidance provided to individuals in a self-directed process. The CPSS staff will build on strengths, identify challenges and help to develop a plan of action. The staff will support all who wish to become better advocates for
themselves and family members.


CPSS is a DRUG FREE program promoting wellness and recovery and works to assist individuals with ways to cope with issues. Staff are peer support individuals who understand through “lived experiences.”

PARTICIPANT GUIDELINES

- Currently re-entering into the community from incarceration


- Released from incarceration within the past 1 year


- Currently working with Parole, Probation or Alternatives Sanctions


- Currently has criminal charges pending

The program is staffed by peers who are there to assist with issues of:

Mental Health, Behavioral Health, Substance Use Disorders, Loneliness/Helplessness

The program provides a network and programming to assist adults on the road to recovery and works to promote wellness in our vision of peer support and advocacy.

THERE IS NO CHARGE FOR ANY OF THE SERVICES PROVIDED.
